How to submit a Statement Information request

1. Open the Submit a Request form and choose “Statement Information”.
2. Enter your email address.
3. Enter the first 6 digits of the card.
4. Enter the last 4 digits of the card.
5. Ask your question in the description field and submit the form.

The form asks only for these partial card details. Never send your full card number, expiry date, or security code. Our support team replies to the email address you enter on the form.
